Job summary

Qualifications

Responsibilities

• Triage patients with acute and chronic needs via telephone or electronic communication and determine next steps.
• Document encounters in the EHR in a timely manner for follow-up and gap closure.
• Coordinate care through multiple service lines and departments.
• Assist with paperwork involved in care coordination.
