Join a team of dedicated service providers who seek to promote the accessibility of services that improve quality of life and support self-determination for persons with mental and developmental disorders. Texoma Community Center (TCC) is one of 39 Texas Community Centers, which are governmental entities as defined by Title 7 of the Texas Health and Safety Code.
Initiates preliminary TRRs and recovery plans for all consumers seen in triage within 7 days, in accordance with CCBHC guidelines. Establishes new consumers coming into services and facilitates transition into the case management team. Provides rehabilitation case management or service coordination services as outlined in the Texas Administrative Code and current HHSC guidelines for persons with persistent and severe mental illness including advocacy, crisis services, individual and group skills training in accordance with the philosophy, goals and objectives of Texoma Community Center. Other duties as assigned. Operates under trauma-informed and recovery-oriented models of care which align with CCBHC standards.
