Job summary

Texas Panhandle Centers is seeking a Case Manager - CFC Service Coordinator in Amarillo, TX. You will manage caseloads, coordinate services, and work within a Trauma-Informed framework to empower individuals with IDD and their families.

The role requires a Bachelor’s degree and case management experience; travel may be involved to support services across counties. Competitive salary with benefits and growth opportunities are provided.

Qualifications

  • Bachelor's degree required.
  • For Case Manager II, minimum one (1) year of experience in case management.
  • For Case Manager III, minimum two (2) years of experience in case management.
  • Graduate work may be substituted for up to one (1) year of experience.
  • Good writing and verbal skills.

Responsibilities

  • Maintain ethical and professional communications using Trauma-Informed Care language.
  • Meet with individuals on assigned caseload at least monthly to monitor services requested.
  • Coordinate with Medicaid provider and state officials to schedule meetings.
  • Lead team meetings to develop the Person Directed Plan annually.
  • Complete community first choice assessments and enter state documents.
  • Travel within the IDD service area as needed to support independence.

Case Manager I:$40,809.60 Annually

Case Manager II: $43,305.60 Annually

Case Manager III: $46,009.60 Annually

Essential duties and responsibilities:
  • Staff will maintain ethical and professional communications at all times using respectful Trauma-Informed Care language
  • Making lives better for individuals with Intellectual Developmental Disabilities both in their homes and in their community
  • Meet with individuals on assigned caseload at least monthly to monitor services requested that may include day habilitation, training in the home, transportation, respite, nursing, dental, and group home/host home
  • Community First Choice is a Medicaid-funded service that offers paid hours to teach individuals daily living skills based on their needs. This service is offered to all general revenue clients who have Medicaid and offers our individuals a chance to retain their independence
  • Lead team meetings to develop the Person Directed Plan annually as well as coordinate with Medicaid provider representative to complete annual joint meetings for community first choice services
  • Complete community first choice assessments, enter state documents, coordinate with Medicaid provider and state officials to schedule meetings in a timely manner
  • Link in the community as needed to aid individuals with living in the community as independently as possible
  • Respond to crisis situations
  • IDD serves individuals in the top 26 counties so some travel may be required
  • Adheres to TPC Business Code of Conduct and reports concerns if ethical and/or business standards defined in the code are compromised
Required qualifications to be successful in the job:
  • Bachelor's degree from an accredited college or university
  • For Case Manager II, minimum one (1) year of experience in case management
  • For Case Manager III, minimum two (2) years of experience in case management
  • Graduate work may be substituted for up to one (1) year of experience
  • Good computer skills including Microsoft Word and Outlook
  • Good writing and verbal skills

  • Must have a valid Texas driver’s license and be insurable under agency vehicle policy
