Job summary

St Anne’s Community Services in Hull is seeking a Team Leader to lead a busy team within Hull DCA. You will support delivery of safe, person-centred care for clients with complex needs and help manage care plans and staff performance.

You will role model best practice, implement behaviour support strategies and help reduce restrictive practices while maintaining close links to practical care delivery.

Qualifications

  • Minimum 2 years’ experience within the care industry.
  • Experience with the creation and implementation of care plans.
  • QCF level 3 qualification or willingness to pursue.

Responsibilities

  • Support the manager with staff recruitment, development and supervision.
  • Ensure service quality and compliance with policies and procedures.
  • Maintain the Care Management System (Access) and manage care plans.
  • Conduct staff supervisions and annual appraisals.
  • Carry out spot checks and monitoring for quality and safety.
  • Support safeguarding and report concerns to management.

We are looking for a client focussed and compassionate Team Leader to support the delivery of safe, person-centred support for clients with complex needs, including learning disabilities, mental health and behaviours that may challenge.

As part of the management team, you will provide visible, hands‑on leadership, supporting colleagues to deliver high‑quality care in line with the PROUD values and the quality standards.

You will role model best practice, implement specialist behaviour support strategies, help manage periods of crisis or escalation, and maintain a clear focus on reducing restrictive practice.

The role also includes direct support duties, helping you stay close to practice and support quality support where it matters most.

You will bring experience in adult social care, strong communication skills, resilience and the confidence to lead by example in a busy and rewarding environment.

Main duties of the job

No two days in this role will be the same, the Team Leader role will involve:

  • Effective Time Management: ability to manage diaries and day to day administration requirements.
  • Teamwork and Adaptability: working closely with Deputy Managers and staff teams, delivering quality support and service. Flexibility to cover additional tasks in line with business need.
  • Care Plan Management: ensuring Care Plans are accurate, reviewed and risk assessed as required with clear communication to care teams and input onto Care Management system.
  • Supervisions: conduct supervisions with all team members and ensure they are documented accurately. Annual Appraisals will also be conducted.
  • Spot Checks: you will be responsible to carry these visits out assessing both team members, clients and ensuring these visits are recorded accurately.
  • Safeguarding: you are responsible for reporting and escalating any concerns, misconduct or complaints to local management and external agencies
  • Continued learning on industry standards and guidance is maintained.
  • Maintain and update Care Management System (Access)
  • KPI’s measured against quality and delivery against industry standards.

Detailed Job Description And Main Responsibilities
  • Supporting the manager in ensuring that all staff employed within your service are recruited, developed, supported, supervised, trained and managed in an appropriate and professional manner.
  • Addressing the training and development needs of yourself and staff and ensuring that they are reviewed against St. Anne’s service objected.
  • Maintaining the quality of the service provided within your defined service.
  • Ensuring that you and staff within your section comply with St. Anne’s values and beliefs, policies and procedures. In addition that any legal or contractual obligations are fully met.
  • Ensuring that your service(s) have access to, and use the best current thinking and practice in the delivery of those services.
  • Contributing to the successful future development of St. Anne’s by highlighting opportunities within the broad corporate aims of the organisation. This will include contributions to policy and procedures and to service developments.
  • Monitoring your service(s) in such a way that timely corrections are possible.
  • Establishing and maintaining effective links within your teams, with other organisations, and agencies or workers whose role(s) may impact upon your service area.
Person specification
Experience

Essential criteria

  • Minimum 2 years’ experience within the care industry
  • Experience with the creation and implementation of care plans

Desirable criteria

  • QCF level 3 (minimum) qualification would be expected or willing to work towards
  • Social Care Supervisor or Team Leader: 1 year (preferred)
Knowledge

Essential criteria

  • Competent with IT Packages

Desirable criteria

  • Understanding how to apply industry standards and guidance.
