Trusted Ally Home Care seeks a Home Health RN Team Lead to supervise care teams, coordinate clinical assessments, and ensure quality patient outcomes in patients' homes.

You will provide direct patient care with one patient in Lakewood, CO on Tue/Wed/Thu 7 am–7 pm, while mentoring staff and supporting operational excellence. A bonus is offered at 30/60 days and six months.

Qualifications

• Be a registered nurse (R.N.) with a current license in the state of practice.
• Current Basic Life Support Certification.
• Minimum of 2-3 years of nursing experience; at least 1 year in home health or community health; leadership experience is a plus.
• EHRs and medical documentation systems proficiency.
• Valid driver’s license and access to a reliable vehicle.

Responsibilities

• Oversee home health operations and coordinate a team of nurses and clinicians.
• Provide direct patient care with a single patient in Lakewood, CO two days a week.
• Lead clinical assessments, care coordination, and documentation.
• Ensure quality, compliance, and emergency readiness across care teams.

Job Summary:
The Home Health Registered Nurse Team Lead will oversee home health operations in our care teams, coordinating and managing a group of nurses and clinicians providing skilled nursing care to patients in their homes. This role involves a blend of clinical expertise, leadership capabilities, comprehensive patient assessments, and effective communication skills to ensure optimal patient outcomes. As a Team Lead, you will be providing direct patient care with a single patient in Lakewood, CO on Tuesdays, Wednesdays, and Thursdays, 7 am - 7 pm.
