Job summary

Liaisons Community Care is seeking a fully licensed clinician to lead a Community Support Team serving adults in Duplin County. The Team Lead drives service delivery, provides direct clinical interventions and individual therapy to every consumer on the team caseload, supervises CST staff, and ensures services remain person-centered, clinically appropriate, coordinated, and compliant.

Qualifications

• Licensed clinical practice in NC with eligible license types.
• Minimum one year experience serving adults with mental health/substance use.
• Strong assessment, therapy, supervision and care coordination skills.
• Knowledge of Medicaid documentation and multi-system coordination.

Responsibilities

• Direct and coordinate CST services as a single clinical team.
• Provide direct clinical interventions and individual therapy to consumers.
• Assess needs and complete functional assessments.
• Supervise CST members and maintain individualized supervision plans.
• Lead weekly team meetings and monitor consumer status and needs.
• Allocate caseloads based on acuity and staffing ratios.
• Develop and revise person-centered plans and recovery-focused interventions.
• Coordinate behavioral health, medical, housing, crisis, and community services.
• Monitor quality, documentation, and CST fidelity; participate in 24/7 crisis coverage.
• Ensure Medicaid, Trillium, CARF compliance and timely documentation.
