Job summary

Centerstone seeks a leader to provide administrative supervision for a Case Management and Health Link team, including hiring and training staff, and conducting monthly chart audits to ensure quality and compliance.

You will coach staff in delivering effective care coordination, maintaining strong documentation practices, and navigating the electronic health record system. Travel between Shelbyville and Fayetteville, TN is required.

Qualifications

• Master’s degree in counseling, psychology, social work, or related field preferred.
• Candidates with a bachelor’s degree and supervisory experience may be considered.
