Job summary

Centerstone, a nonprofit behavioral health system in Tennessee, seeks a Director of Case Management and Health Link to lead clinics in Shelbyville and Fayetteville. You will hire, train, and supervise staff, drive chart audits, ensure quality documentation, and promote effective care coordination across EHRs.

Qualifications

• Master’s degree preferred in counseling, psychology, social work, or related field.
• Bachelor’s degree with two years’ case management and one year supervisory experience may be accepted in lieu of a Master’s.

Responsibilities

• Provide direct administrative supervision to Case Management and Health Link team.
• Hire and train staff; conduct monthly chart audits and ensure quality documentation.
• Coach staff in care coordination, promoting health and wellness, and navigating EHR systems.
• Develop staff and strengthen service delivery to improve client access to support.
