Qualifications

• Associate’s Degree in healthcare or equivalent required.
• Bachelor’s degree preferred.
• Minimum of three years of clinical experience required.
• Three years of long-term care experience preferred.
• Managed Care/Medicare/Medicaid experience preferred.

Responsibilities

• Provide feedback to staff regarding performance outcomes and goals.
• Monitor work allocation to maximize output and effectiveness.
• Oversee scheduling and attendance of personnel.
• Audit staff performance and provide coaching.
• Conduct regular one-on-one meetings with personnel.
• Coordinate team-specific orientation and training.
• Triaging case management assignments to facilitate CM process.
• Serve as liaison for CM/Outreach services issues.
• Network with other departments to share information.
• Public speaking and ongoing staff training guidance.
• Assist with development of CM/Outreach processes for cost-effective care.
• Regularly oversee community-based contacts and visits.
