RICHMOND RESIDENTIAL SERVICES, INCORPORATED is seeking a Team Case Manager to provide case management for adults with intellectual disabilities in community settings. The role involves some direct care, data management, and coordination with supervisors to ensure high quality, person-centered supports.

You will manage a caseload, document in the EHR, assist with meetings and care planning, participate in on-call duties, and help maintain regulatory compliance with DBHDS standards.

Qualifications

• Experience in case management for adults with disabilities.
• Strong communication and documentation skills.
• Ability to interpret DBHDS standards.

Responsibilities
