Damar Services, Inc. in Richmond, Indiana is seeking a dedicated Case Manager to lead and monitor treatment services for individuals with DD/ID. You will develop IPPs, coordinate with Residential Managers, and ensure documentation and communication with families, clinicians, and schools.

Qualifications

• Must have at least one year of experience in treating DD/ID population or specialized training.
• Minimum age of 21 and valid Indiana driver’s license if driving is required.
• Clear of felony convictions and pass pre-employment drug screen and physical.
• Ability to develop IPPs within 7 days of admission and update as needed.

Responsibilities

• Lead, direct and monitor treatment services for assigned clients.
• Develop Individualized Program Plans (IPPs) within 7 days of admission.
• Collaborate with Residential Managers to ensure unit programming.
• Write and update monthly IPPs and review MHA/addendum.
• Conduct ICAP assessments and Ansell Casey evaluations as required.
• Maintain service notes, data sheets and documentation for clients.
• Represent clients at court reviews, IEP/case conferences and attend meetings.
