Richmond Residential Services, Incorporated is seeking a Team Case Manager to oversee a caseload of adults with intellectual disabilities in community-based settings. This full-time role in Henrico, VA includes direct care duties, quarterly reports, risk management, and EHR documentation.

Supervision comes from Program Supervisors/Directors; a sign-on bonus of $1000 and a $1.50/hour shift differential apply.

Qualifikationen

• Bachelor's degree in Human Services or related field required or preferred.
• One or more years of directly related experience or any combination of training and experience.
• Valid Virginia Driver's license and acceptable driving record.
• Ability to pass a background check and screenings.

Aufgaben

• Manages a case load of 7-10 individuals to complete quarterly reports and assessments.
• Maintains electronic health care records in the EHR system.
• Supports annual meetings and root cause analysis for assigned caseloads.
• Provides direct care support when not performing case management duties.
• Participates in On-Call responsibilities for the director/team and responds to care needs.
• Oversees program health appointment and records system.
• Communicates and advocates for clients per policy and regulations.
• Maintains compliance with licensing standards and Human Rights regulations.
• Performs other related duties as assigned.
