Qualifications

JOB DESCRIPTION
TITLE: TeamCase Manager

This position is eligible for a sign-on bonusof $1000 (with stipulations).

RRSI offers a shift differential of $1.50 per hour (in addition to thehourlybase rate) for all hours worked from 4P to 7A.

GENERAL STATEMENT OF RESPONSIBILITIES :

Provides case management support for adults with intellectual disabilities living incommunity-basedsettings. Will provide some direct care and other risk management functions according to the need presented. Supports Program Supervisors/Directors to enhance the quality of care through individual choicein accordance withthe agency’s mission. Performs duties with focus on person-centered services, choice, discretion,risk-management and attention to detailin accordance withthe agency’s mission.

Receives supervision from Program Supervisors/Directors, who provide specific and general assignments and review work asappropriate interms of final results .

ESSENTIAL DUTIES & RESPONSIBILITIES include but are not limited to:
  1. Manages a case load of7-10 individuals to complete quarterly reports, plan updates, fall risk plans/assessments, annual paperwork, support instructions, and assessment updates as needed.
  2. Maintainsthe electronic health care record for individuals assigned to ensure that all data is uploaded into the EHR system.
  3. Provides support at annual meetings, client care specific meetings, and supports the completion of root cause analysis for individuals on assigned caseloads.
  4. Provides some direct care support to individuals at assigned programs when not engaged in case management duties. This may include support for medical appointments, community activities, and/or DSP coverage in assigned programs.
  5. Willparticipatein On-Call responsibilities for assigned RRSI Director,Team, and respond to individual care needs and or critical staffing issues as needed.
  6. Provides oversight for assigned program health appointment and records system.
  7. Professionally communicates and advocates for clientsin accordance withRRSI Code of Ethics and all applicable DBHDS and Human Rights regulations.
  8. Assists assigned Program Supervisor/Director withmaintainingcompliance for all DBHDS Licensing standards and Human Rights compliance.
  9. Performs other related duties as assigned.
KNOWLEDGE, SKILLS & ABILITIES:

Knowledge: Strong knowledge of intellectual disability concepts,principlesof normalization, and independent living needs of adults with intellectual and/or developmental disabilities. Knowledge and commitment to person-centered thinking, ability to make individualchoices, and positive behavioral support. Knowledge of current DBHDS standards for person-centered supports, risk management strategies, and behavioral management competencies as outlined by DBHDS.

Skill: Excellent communication skills to communicate effectively in written and oral form. Effective organizational and time management skills. Computer skills, witha high levelofproficiencyin database programs and attention to detail tomaintainaccuraterecordkeeping. Working knowledge of other primary Microsoft Office programs. Safe operation of a passenger vehicle.

Ability:Demonstratedability tomaintainclient recordsin accordance withRRSI policy, DBHDS andDMAS standards and manage time effectively to meet deadlines. Able to interpret agency policies and procedures whenmaintainingclient records. Able to provide case management support, understand risk mitigation strategies, and carry out emergency procedures when needed. maintainappropriate CPR/FA/AEDcertification; interact effectively and courteously with team members, clients, and the public. Able to be flexible in work scheduling (which includes working at least two weekends per month). Dedication to keeping up to date technically, using technology effectively, and applying new knowledge to the job.

QUALIFICATIONS:
  • Bachelor's Degree in Human Services preferred, or a field related to community services.
  • One or more years ofdirectly relatedexperience or any combination of training and experience which provides the required knowledge, skills, and abilities.
  • Valid Virginia Driver's license and acceptable Department of Motor Vehicles Driving record and dependable transportation.
  • Ability to successfully pass a background check, and other pre-employmentscreenings.

  • Able to lift up to 50 pounds toassistindividuals who have physical challenges with positioning, transferringor ambulating as needed.
  • Occasionally bend, kneel,stoopand lift toassistclients with tasks such as walking, dressing,toiletingand bathing.

WORK ENVIRONMENT:

The work environment is characteristic of a standard office environment. Electrical hazards related to frequent contact with office equipment. Occasionally,the workenvironment may be a home or apartment-like setting. Outside conditions exist when traveling to and from activities and meetings.
