Job description

We are hiring for: Clinical Case Manager (Regular).

Salary: $55,000–$65,000 annually.

Reports to: IDD Group Homes Program Manager.

Supervisory Responsibilities

This position has no supervisory responsibilities.

Duties & Responsibilities
  • Assist with coordinating Interdisciplinary Team (IDT) meetings and Individual Support Plans (ISPs).
  • Supervise and monitor the implementation of ISPs.
  • Ensure a streamlined admission process.
  • Respond to incident reports, develop root‑cause analyses and response plans.
  • Analyze and summarize progress status, determine program appropriateness and recommend changes.
  • Communicate regularly with the Support Coordinator and other members of the circle of support regarding ISP amendments; document all communication.
  • Spend time at the location where services are performed and conduct announced and unannounced visits in residential homes during each shift to ensure policy, procedures, and program data are in place.
  • Provide coverage for shortages of direct support personnel while maintaining management responsibilities.
  • Assist in on‑call rotation, including weekends and holidays, as specified by the Administrator.
  • Train and conduct in‑services for direct care staff as needed.
  • Conduct internal peer reviews, observations, and other quality assurance assessments.
  • Conduct CQL’s Personal Outcome Measures.
  • Support key components of the person’s health record for accuracy and timeliness.
  • Perform all other duties as assigned.
Minimum Qualifications
  • Bachelor’s degree in Social Work, Social Services, Human Services or Special Education required.
  • Master’s degree a plus.
  • Minimum 2 years of experience working with persons with intellectual and/or developmental disabilities (1 year if paired with a Master’s degree).
  • Working knowledge of applicable regulations related to our service offerings.
  • Valid driver’s license, auto insurance, and reliable transportation.
  • Supervisory/management experience and experience writing person‑centered plans preferred.
  • Must be at least 18 years of age.
Physical Demands and Work Environment
  • Regularly required to lift 30 lbs; must be able to lift at least 15 lbs.
  • Must be able to pull a minimum of 30 lbs.
  • Must be able to squat, kneel, crawl, crouch, climb, and stoop.
  • Must be able to regularly use hands to finger, handle, or feel objects, tools, or controls.
  • Required to regularly stand and walk.
  • Must demonstrate proficiency in CPR from the floor level, including work on hands, knees, bending, standing, and lifting.
  • Vision requirements include close vision, distance vision, and peripheral vision.
  • Must be able to talk and hear.
