Job summary

Community Youth Services is seeking a dedicated WISe Care Coordinator to support Transitional Age Youth through intensive, community-based care coordination. This role navigates behavioral health, educational, legal, and community systems to build skills and supports for long-term success.

The position offers $34.75 - $44.55 per hour for a full-time, 40-hour work week, non-exempt status, serving Thurston and Mason Counties in Washington.

Qualifications

• Bachelor’s degree preferred; equivalent experience may be considered.
• Ability to obtain or maintain WA Agency Affiliated Counselor credential.
• Experience working with youth and families in behavioral health or social services.
• Strong communication, organization, and relationship-building skills.

Responsibilities

• Provide intensive care coordination and case management services.
• Facilitate Child and Family Team meetings.
• Conduct assessments and develop individualized plans of care.
• Coordinate services across behavioral health, schools, DCYF, juvenile justice, and other systems.
• Collaborate with Youth and Family Peer Specialists.
• Maintain accurate documentation and track participant progress.
• Support youth and families through all phases of WISe services.
