Coastal Horizons Center in Craven and Pamlico Counties (New Bern, NC) is seeking a dedicated TASC Care Manager to join our team. This role focuses on case management and service coordination within the TASC program for justice-involved individuals.

You will administer psychosocial screenings, coordinate linkage to behavioral health, medical, educational, and vocational services, and support clients with ongoing care management.

Qualifications

• Bachelor's degree or higher in human services or related field.
• NC Peer Support Specialist certification preferred and/or NCASPPB registration.
• Knowledge of recovery, harm reduction, and wellness concepts; lived experience is a plus.

Job Title: TASC Care Manager

Location: Craven & Pamlico Counties (New Bern, NC)

Hours: 40 hours/week, Full Time, M-F, Non-Exempt

Primary Office: Craven County; Secondary Office: Pamlico County

Coastal Horizons Center is seeking a dedicated and qualified TASC Care Manager to join our team. This position offers a unique opportunity to support justice-involved individuals by providing case management and service coordination within the TASC (Treatment Accountability for Safer Communities) program. The ideal candidate will collaborate with community partners, justice systems, and service providers to help individuals access treatment and recovery services.

Primary Duties

Administer psychosocial screenings/assessments; coordinate linkage to behavioral health treatment, as well as medical, educational, and vocational services as needed; and provide ongoing professional care management services for the justice-involved population.

Qualifications

• Education: Bachelor's Degree or higher in a human services or related field
• Certifications: Preference given to individuals with North Carolina Peer Support Specialist certification and a minimum of 2 years of experience, and/or individuals registered with the North Carolina Addiction Specialist Professional Practice Board (NCASPPB)
• Experience: Knowledge of recovery, harm reduction, and wellness-based concepts. May have lived experience with recovery and/or the justice system.

Required Skills

• Strong communication, organizational, and time-management skills.
• Ability to work independently and manage a caseload effectively.
• Proficiency in the Microsoft Office Suite and similar platforms.
• Proficiency in using electronic health record systems and remote collaboration tools (i.e., Teams, Zoom, etc.).
• Demonstrated ability to collaborate with diverse stakeholders, including legal systems, healthcare providers, and social services.
