Job summary

Coastal Horizons Center, Inc. is seeking a dedicated TASC Care Manager to provide case management and service coordination for justice-involved individuals. This full-time position will require you to administer assessments, coordinate treatment resources, and collaborate with community partners.

The ideal candidate will hold a Bachelor’s Degree or higher in a related field and demonstrate strong communication and time-management skills. A competitive salary ranging from $42,000 to $45,000, along with comprehensive benefits, will be offered.

• Experience with legal systems or as a Peer Support Specialist preferred.

Hours

40 hours/week, Full Time, M-F, Non-Exempt

Position

Coastal Horizons Center is seeking a dedicated and qualified TASC Care Manager to join our team. This position offers a unique opportunity to support justice-involved individuals by providing case management and service coordination within the TASC (Treatment Accountability for Safer Communities) program. The ideal candidate will collaborate with community partners, justice systems, and service providers to help individuals access treatment and recovery services.

Location

Carteret County (Morehead City, NC)

Primary Duties

Administer psychosocial screenings/assessments; coordinate linkage to behavioral health treatment, medical, educational, and vocational services as needed; provide ongoing professional care management services for the justice-involved population.

Qualifications

• Education: Bachelor's Degree or higher in a human services or related field
• Certifications: Preference given to individuals with North Carolina Peer Support Specialist certification and a minimum of 2 years of experience, and/or individuals registered with the North Carolina Addiction Specialist Professional Practice Board (NCASPPB).
• Experience: Knowledge of recovery, harm reduction, and wellness-based concepts. May have lived experience with recovery and/or the justice system.

Required Skills

• Strong communication, organizational, and time-management skills.
• Ability to work independently and manage a caseload effectively.
• Proficiency in the Microsoft Office Suite and similar platforms.
• Proficiency in using electronic health record systems and remote collaboration tools (i.e., Teams, Zoom, etc.).
• Demonstrated ability to collaborate with diverse stakeholders, including legal systems, healthcare providers, and social services.
