Job summary

Life’sWork of Western PA is seeking a compassionate, organized Case Manager to provide in-home case management to children, youth, and families. You will partner with referral sources, families, community providers, and county agencies to support positive outcomes and connect clients with resources.

The role involves regular in-person visits, service planning, care coordination, advocacy, and meticulous documentation, with nontraditional hours as needed and a daily caseload of 2–3 clients.

Qualifications

• Bachelor’s degree in social work or related field; Master’s degree preferred; Associate degree with 3–5 years of experience.
• Must have a valid Pennsylvania Driver’s License, reliable vehicle, and current auto insurance.
• Act 33/34, Criminal Background, and FBI clearances are required.
• Flexibility for nontraditional hours (evenings/weekends) and ability to transport clients.
• Strong verbal and written communication, confidentiality, and professional demeanor.

Responsibilities

• Provide in-home case management visits based on referral needs and treatment goals.
• Develop service plans, coordinate care, advocate for clients and navigate systems.
• Connect clients with housing, mental health, substance use treatment, employment and education supports.
• Complete assessments, releases, background information, and required intake documentation.
• Maintain client files and meet deadlines in Horizon system.
• Transport clients to appointments and collaborate with CYF Case Workers and partners.
