Job summary

People Incorporated Mental Health Services in Dakota County, Minnesota is seeking a Case Manager I or II to support clients with chronic mental illness, developing person-centered plans and coordinating access to services.

Qualifications

• Bachelor’s degree in the behavioral sciences or related field (including social work, psychology, or nursing) from an accredited college or university.
• Master’s degree in behavioral science or related field qualifies for Case Manager II.

Responsibilities

• Perform functional assessments that lead to individualized plan development and access to mental health services.
• Conduct regular reviews of plan effectiveness and adjust services to maximize support.
• Assess client needs and ensure continuity of care; make referrals based on assessment.
• Maintain daily records and document interventions consistent with standards.
