People Incorporated Mental Health Services seeks a Case Manager II for its Targeted Case Management team in the Metro County area of Minnesota. The role focuses on clients with chronic mental illness, developing person-centered plans to maximize independence and community integration.

Hybrid work in the Twin Cities metro with full-time hours, monthly planning meetings, and a focus on autonomy and client advocacy.

Qualifications

• Bachelor’s degree in behavioral science or related field or Master’s degree with required supervised experience.
• 2,000 hours of supervised experience working with adults with severe and persistent mental illness.
• Master’s degree in behavioral science or related field preferred.

Responsibilities

• Perform functional assessments leading to individualized plan development.
• Review plan effectiveness and adjust to maximize support and housing access.
• Maintain daily records for continuity of service and reporting.
• Document interventions and meetings in the treatment plan per standards.

Case Manager II - Targeted Case Management (Metro County)People Incorporated is seeking a Case Manager II to join our Targeted Case Management (Metro County) team to primarily work with clients who are diagnosed with chronic mental illness or other chronic health conditions. We connect people with services through developing person-centered support plans that help them live as independently as possible. Our Case Managers develop long term relationships with their clients and operate autonomously, creating their own schedules.Hours: Full - Time, Monday - Friday, business hoursLocation: Metro County (Twin Cities) , Minnesota l HybridRange: $24.25 - $25.75 Hourly, based on qualificationsHiring Bonus: $1,000 to external candidatesPeople Incorporated Mental Health Services is the largest nonprofit mental health provider in Minnesota. For over 50 years, we have been a leading community partner, transforming the health of our communities through innovative solutions.Main Job Duties:• Perform functional assessments that lead to individual plan development and result in the client receiving access to necessary mental health and related services.• Conduct regular review of plan effectiveness and make adjustments as needed that result in the client receiving the maximum amount of assistance and support while monitoring the effectiveness of services received.• Assessing client needs and ensuring continuity of care.• Making ongoing referrals to resources based on assessment and current client need.• Maintaining necessary daily records for continuity of service and reports to funding agencies.• Excels at documenting all interventions, interactions and relevant information to the treatment plan consistent with professional and organizational standards.• Attending client monthly meetings to monitor services.To perform this job successfully an individual must be able to perform each essential duty satisfactorily.
