Qualifications

• Bachelor's degree in psychology, social work, or mental health.
• Minimum one year of experience post degree in human services.
• Must provide own transportation to provide home visits.

Responsibilities

• Conducts home visits to assess service coordination needs for clients.
• Assists clients in accessing all needed services within the community.
• Performs advocacy on behalf of clients.
• Provides case consultations to service providers.
• Provides pre-tenancy and tenancy supports to assist participants in identifying, selecting, and entering into a lease.
