Job summary

Kids for the Future in Elizabethtown, KY is seeking a Home Services Coordinator to provide service coordination for targeted populations through home visits. Responsibilities

POSITIONSUMMARY:

Providesservicecoordinationtotargetedpopulationclients(Childor Adult).

QUALIFICATIONS:

Bachelor’s degree in psychology, social work, or mental health; preference for work experience with the targeted population assigned within the human service setting. Minimum one (1) year of experience post degree working with persons in a human services setting. Must provide own transportation to provide home visits.

SKILL SETS:

PHYSICAL AND ENVIORMENTAL DEMANDS:

Visual acuity necessary for updating files and completing documents. Requiresfrequentstooping,kneelingandcrouchingto filerecords. Hearingandtalkingisnecessary toanswerphoneandtalkwithemployeesandotherindividuals. Workislightwithwalking,standingandsitting. Manual dexterity is required to perform work processing and other clerical duties. Possible exposure to blood borne pathogens, biohazardous waste, hazardous waste and communicable diseases.
