Job summary

KVC Kentucky is seeking a Family Care Coordinator / Case Manager to provide in-home behavioral health services to children and families in Lexington, KY. The role requires a Bachelor’s degree in Social Work or related field and experience with complex families.

You will work with DCBS, community partners, and a dedicated team, ensuring safety plans and ongoing communication. The position emphasizes strong collaboration, a flexible schedule, and adherence to agency policies.

Qualifications

• Bachelor’s degree in Social Work (or related field) required.
• Experience delivering in-home services to children and families.
• Ability to work with complex, multi-need families and coordinate with DCBS.
• Familiar with safety planning and action planning.

Responsibilities

• Complete face-to-face intake with family within 24 hours to assess safety and service needs.
• Develop and maintain a Safety Plan and share with the family.
• Act as the main contact for the family and DCBS throughout the intervention.
• Coordinate with providers and community resources to meet identified needs.
• Schedule and conduct team meetings to review progress and goals.
• Document case activity and ensure timely submission in ChartOnline.
