Job summary

Westmoreland Casemanagement And Supports Inc in Greensburg, PA seeks a full-time Targeted Case Manager to assess client needs, develop a comprehensive service plan, and coordinate care across child, transitional age, and adult populations.

You will work in an interdisciplinary team, manage records, participate in outreach events, and provide crisis intervention and advocacy to improve housing, education, and employment outcomes.

Qualifications

• Bachelor’s degree in social services related field; casemanagement experience preferred.
• Two years’ direct contact with Behavioral Health clients and two years as a TCM Level 1 at WCSI.
• Strong interpersonal, oral and written communication skills.

Responsibilities

• Assess client needs using the Environmental Matrix and SBA; interviews and records review.
• Develop, prepare, and implement a comprehensive Targeted Case Management Service Plan.
• Monitor services, coordinate with providers, and advocate for clients.
• Respond to crises; participate in after-hours on-call rotation and outreach events.
• Maintain case records and prepare for audits; ensure regulatory compliance.
