Job summary

Westmoreland Casemanagement And Supports Inc is seeking a full-time Targeted Case Manager to assess client functioning across child, transitional age, and adult populations, and to develop and implement comprehensive service plans with housing, vocational, and educational goals.

The role involves on-site work with occasional evenings/weekends for after-hours on-call rotation, collaboration with an interdisciplinary team, and thorough documentation in a flexible, community-based environment.

Qualifications

• Bachelor’s degree in a social services related field.
• Previous casemanagement experience preferred.
• Associate degree in Human Service/Social Science and two years’ direct Behavioral Health experience and two years as a TCM Level 1 at WCSI.
• High school diploma with 12 semester credits in sociology, social welfare, psychology, gerontology, or related field and two years of mental health experience with two years as a TCM Level 1 at WCSI.

Responsibilities

• Assess client needs using Environmental Matrix and SBA and reviews with client/collateral.
• Develop, prepare, and implement a comprehensive Targeted Case Management Service Plan with goals and outcomes.
• Monitor all services, coordinate resources, advocate for clients, and attend team meetings.
• Respond to crisis needs; participate in 24-hour on-call rotation when scheduled.
• Maintain documentation, complete charts, and ensure program compliance.
• Engage in community outreach, represent the organization, and maintain resource directory.
