Job summary

Southwest Behavioral Care, Inc. is seeking a full-time case manager to conduct level of care assessments and coordinate referrals for adolescents in Westmoreland County schools and adults in office/community settings in Greensburg, PA.

You will travel to school districts, maintain client records, and collaborate with referral sources to support access to treatment and recovery. On-call rotations and compliance with regulations are required.

Qualifications

• Bachelor's degree in Social Work, Psychology, Sociology, Nursing, Chemical Dependency, Criminal Justice, or related field.
• Ability to obtain vital information during interviews to make proper service recommendations.
• Knowledge of mental health programs and available social services.

Responsibilities

• Conduct level of care assessments and coordinate referrals and transportation as needed.
• Link adolescents to school-based services and adults to community services.
• Travel to Westmoreland County school districts for adolescent services; conduct adult services in office/community.
• Maintain contacts with referral sources and school personnel within the county.
• Attend trainings, educational seminars, and staff/community meetings.
• Participate in 24/7 on-call after-hours rotation to assist clients telephonically.
