Qualifications

• Experience in case management or human services is preferred.
• Ability to document services and maintain records clearly.
• Strong communication and advocacy skills.

Responsibilities

• Assess clients comprehensively and document needs.
• Develop client service plans aligned to assessments.
• Arrange and coordinate necessary services identified in assessments.
• Support clients in accessing services through referrals.
• Monitor progress, track appointments, and reassess needs.
• Advocate for clients by expediting entitlements and connecting resources.
• Prepare and maintain case records per standards.
• Provide case consultations with other service providers.
• Communicate with supervisor about case management issues.
• Offer psychosocial education to clients and families.
• Provide crisis intervention and arrange emergency referrals.
• May serve as Primary Case Coordinator depending on role.

Education

Bachelor's degree in Social Work or related field

Completion of state-required case management course
