St Augustine Youth Services in Saint Augustine, FL is seeking a Targeted Case Manager to support children and families in the COACHES and Community-Based Services team. You will assess needs, coordinate services across behavioral health, education, medical, and community resources, and advocate for supports in the community.

Qualifications

  • Bachelor's degree in Social Work, Psychology, Counseling, Human Services, or related field with at least one year of experience with children with serious emotional disturbances.
  • Bachelor's degree in related field plus three years of qualifying experience with children with SERD.
  • Eligible to obtain the CBHCM credential within 30 days of hire.
  • Complete AHCA TCM training within required timeframes.
  • Knowledge of community resources and behavioral health services.
  • Excellent communication, documentation, and organizational skills.
  • Must possess a valid Florida driver's license with acceptable driving record.

Responsibilities

  • Monitor progress and adjust service plans to meet goals.

  • Transport clients to appointments using SAYS vehicle.
