Job summary

SMA Healthcare in Daytona Beach, FL is seeking a dedicated Case Manager to support clients across the full spectrum of the CCBHC criteria, including SMI, SUD, SED, and COD. The role focuses on thorough assessments, service planning, documentation, and coordinated care with a broad care team.

You will document in SMA’s EMR, facilitate connections to community resources, and advocate for informed treatment options while ensuring safety, quality, and compliance with program guidelines.

Qualifications

  • Bachelor's degree in a human services field is required or preferred with relevant experience.
  • Experience working with mental illness, SUD, and related populations is favored.
  • Certification as a Targeted Case Manager may be required within six months.

Responsibilities

  • Assess clients and develop service plans to address goals and needs.
  • Complete authorizations and re-authorizations per guidelines.
  • Coordinate care and monitor service delivery with the interdisciplinary team.
  • Document activity in SMA's EMR and ensure accurate records.
  • Facilitate access to community resources and advocate for clients.

Essential Job Functions:
  • Serving children and adults identified within the scope of theCCBHC criteria ie individuals with severe mental illness(SMI), substanceuse disorder(SUD), children and adolescents with severe emotionaldisturbances (SED); individuals with co-occurring disorders (COD) andthose experiencing a mental health or substance use related crisis.
  • Completesassessments and service plans that assist clients in developing goalsand addressing areas of need and reviews the assessments and serviceplans according to program guidelines.
  • For children andadolescents, this plan is to be created in collaboration with theparents, Guardian Ad Litem, foster parents, and other pertinent parties.
  • Completes authorizations and re-authorizations as required by program guidelines.
  • Participatesin client treatment planning and case review with other client careproviders including but not limited to CPC, DCF, dependency, juveniledetention services as it relates to children and adolescents.
  • Documentsservice activity in SMA's EMR, and ensures the medical record is up todate, accurate, and meets all documentation standards in accordance withprogram requirements.
  • Coordinates and provides care that is safe, timely, effective, efficient, equitable, and client-centered.
  • Executes linkages identified on the service plan and monitor progress of service delivery according to program guidelines.
  • Facilitatesclient access to community resources, including locating recreationalactivities, housing, food, clothing, school programs, vocationalopportunities or services, providers to teach life skills, and relevantmental health and substance use services.
  • Assists clients with identifying financial assistance options for medications and other medical needs.
  • Assists clients with developing natural resources and making contact with social support networks.
  • Helps clients make informed decisions by acting as their advocate regarding their clinical status and treatment options.
  • Communicatesclients' progress by conducting/attending interdisciplinary meetingsand evaluations; disseminating results and obstacles to therapeutic teamand family; and identifying treatment influences.
  • Provides limited crisis intervention under appropriate supervision.
  • Reports any concerns regarding the health, safety, or well-being of a client to the appropriate authority.
  • Facilitates access to and/or coordinates transportation for clients in order to execute linkages identified on the service plan.
  • Maintains an awareness of proper safety procedures and guidelines and applies these in performing daily activities and tasks.
  • Providesadministrative support and performs general office duties such asanswering the phone, communicating with internal and external customers,and cleaning as required by program.
  • Adheres to professional standards as outlined by protocols, rules and regulations.
  • Additional duties may be required as per specific program guidelines.
Qualification Requirements:

Minimum Education and/or Experience

: Bachelor's Degree from an accredited university or college with a major in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education, or other related human services field required; a minimum of one year of full time or equivalent experience working with the target population preferred. A Bachelor's Degree from an accredited university or college in a discipline other than previously noted will be considered if the candidate has three years of full time or equivalent experience working with the target population. Preference will be given to those who are certified as Targeted Case Managers. Additional education and/or experience may be required as per specific program guidelines. Certification as a Targeted Case Manager may be required within six months of hire depending on program guidelines.

Knowledge/Skills/and Abilities:
  • Skill in time and organizational management; ability to completework under strict time lines, while maintaining flexibility to assistwith client issues/concerns.
  • Ability to quickly assesssituations and use sound judgment in diffusing potentially riskysituations; ability to identify services needed by clients during therehabilitation process.
  • Ability to follow both oral and written instructions and complete tasks as instructed.
  • Ability to express oneself clearly and concisely both orally and in writing.
  • Ability to prepare clear and concise written reports.
  • Ability to communicate clearly with clients to develop a supportive and trusting relationship.
  • Abilityto establish and maintain effective working relationships withco-workers, medical and clinical professionals, educational systems,court systems, and the general public.
  • Knowledge of community resources.
  • Knowledgeconcerning current information in the area of mental illness andsubstance abuse and treatment and related behavioral problems specificto the age and population served.
  • Knowledge of managed care environment and different payer sources.
Necessary Special Requirements:

Possession of avalid Florida driver's license, acceptable driving record, and proof ofpersonal automobile insurance if required to drive an SMA vehicle and/oruse a personal vehicle for SMA business. Complete State of Floridamandatory background screening prior to start of employment. CompleteSMA required training during the first six (6) months of employment andupdated if required.

Physical

Mobility and ability to bend and reachduring an 8-12 hour day. Able to lift minimum 10 pounds. Visual andauditory acuity sufficient to evaluate, intervene, treat, and recordclient health care needs. Fine motor skills for legible and accuratecharting, daily correspondence and presentation, either manually ororally. Work endurance ability to work 8-12 hour shifts with a mealbreak, as possible. Routine 8-12 hour shifts. Hours and days off mayvary. Extra hours may be required. Work assignment locations may vary.
