St. Augustine Youth Services seeks a Targeted Case Manager for the COACHES Program in St. Augustine, FL.

This full-time, M-F daytime role supports children with serious emotional and behavioral needs, coordinating services with families, schools, and treatment teams. You will complete assessments, develop service plans, and monitor outcomes under Florida Medicaid guidelines, linking families to resources and advocating for needed services.

Qualifications

  • Eligible to obtain the CBHCM credential within 30 days of hire
  • Complete AHCA Targeted Case Management training within required timeframes
  • Knowledge of community resources and behavioral health services
  • Excellent communication, documentation, and organizational skills
  • Valid Florida driver's license with acceptable driving record

Responsibilities

  • Serve as the Targeted Case Manager for approximately 1–3 children
  • Complete assessments and develop, update, and monitor individualized service plans
  • Provide the five core functions: Planning, Assessment, Linking and advocacy, Monitoring, Discharge planning
  • Coordinate services with families, providers, schools, and community partners
  • Attend MDT/IPT, school and provider meetings and document progress
  • Maintain accurate client records and reports
  • Assist with program data and reporting as needed
  • Participate in supervision and team meetings
  • Follow SAYS policies and state/federal requirements

Targeted Case Manager – COACHES ProgramSt. Augustine Youth Services (SAYS) | St. Augustine, FL | Full-Time | M-F daytime hours43680.00 annually plus $4160 in bonusesMake a meaningful difference in the life of a child.St. Augustine Youth Services (SAYS) is seeking a Targeted Case Manager to join our COACHES Program. This position provides targeted case management services to children and adolescents with serious emotional and behavioral needs and works closely with youth, families, treatment teams, schools, and community providers.The Targeted Case Manager plays an important role in helping children and families access the services, resources, and supports they need to achieve their individualized goals.What You'll DoServe as the Targeted Case Manager for approximately 1–3 children.Complete assessments and develop, update, and monitor individualized service plans in accordance with Florida Medicaid requirements.Provide the five core functions of targeted case management:PlanningAssessmentLinking and advocacyMonitoringDischarge planningWork directly with children and families to address barriers and support implementation of their service plans.Connect children and families with appropriate community resources, providers, and services.Advocate for services and resources necessary to meet identified needs.Coordinate services with families, natural supports, treatment providers, schools, and other community partners.Participate in treatment planning and individualized service plan reviews.Attend required meetings, including Multidisciplinary Team (MDT), Integrated Practice Team (IPT), school, and community provider meetings.Monitor service delivery and document progress toward identified goals.Provide support and assistance during times of crisis, including helping families access appropriate resources.Coordinate discharge planning and aftercare/follow-up services.Participate in clinical care coordination and staffing with members of the child's treatment team, which may include physicians, psychiatric providers, therapists, teachers, attorneys, guardians ad litem, and other professionals.Maintain accurate client records, case notes, electronic health records, and other required documentation.Complete progress reports, service plan updates, and other required reports.Maintain knowledge of community resources and referral options.Assist with program data and reporting as needed.Participate in supervision, staff meetings, and treatment team meetings.Follow all SAYS policies, procedures, and applicable state and federal requirements.Perform other duties as assigned..What We're Looking ForRequired QualificationsOne of the following:Bachelor's degree in Social Work, Psychology, Counseling, Human Services, or a related field and at least one year of experience working with children experiencing serious emotional disturbancesORBachelor's degree in a related field plus three years of qualifying experience working with children with serious emotional disturbances.Additionally, candidates must:Be eligible to obtain the Certified Behavioral Health Case Manager (CBHCM) credential within 30 days of hire.Complete required AHCA Targeted Case Management training within required timeframes.Possess knowledge of community resources and behavioral health services.Have excellent communication, documentation, and organizational skills.Maintain a valid Florida driver's license with an acceptable driving record.Why This Work MattersYouth Care Workers are among the people who have the most direct day-to-day contact with the young people we serve.
