Job summary

BestSelf in Buffalo, NY seeks a School Based Targeted Case Manager to support wellness and recovery goals within certified behavioral health sites, coordinating targeted interventions and ensuring timely, high-quality care for youth and families.

You will meet clients at home, coordinate with therapists, maintain detailed records, and help families navigate health and social services, including Medicaid enrollment when eligible.

Qualifications

  • Bachelor's degree in social work, psychology, nursing, rehabilitation, education, occupational therapy, counseling, or community mental health; plus one year direct mental health or substance use experience.
  • Master's degree in one of the listed fields may substitute for one year of experience.
  • Bilingual strongly preferred.
  • Understanding of mental health and substance use disorder diagnosis and symptoms.
  • Familiarity with community resources, entitlement programs, and social services.
  • Ability to broker services independently and develop resources.
  • Must possess valid NYS Driver's License and comply with driving requirements.

Responsibilities

  • Meets with clients and families at homes to review paperwork and obtain consents and treatment plans.
  • Coordinates services, provides psychosocial support, and maintains detailed case records.
  • Refers clients to community resources and coordinates with counselors for effective service delivery.
  • Supports navigation of health care and social service systems.
  • Advocates for individuals and families and assists with Medicaid/Health Home enrollment when eligible.
  • Focuses on HEDIS and population health standards to improve quality of care.
  • Performs follow-up and coordinates with TCM/Clinic and Care Coordination teams.

Targeted Case Manager of School Based Services


Reports To: School Based Program Director

FLSA Status: Non-Exempt

SUMMARY OF DUTIES

The School Based Targeted Case Manager performs activities within the Certified Community Behavioral Health statelite sites that are intended to support the wellness and recovery goals of individuals with complex and/or chronic behavioral health issues and needs by implementing targeted interventions designed to provide timely, high-quality, and efficient care. School Based Targeted Case Management services are organized around goals aimed at providing access to services that encourage individuals to resolve problems that interfere with their attainment or maintenance of independence or self sufficiency, and maintain themselves in the community. Individuals served will be within targeted populations who are not linked to Health Home services.

POSITION RESPONSIBILITIES
  • Meets with clients and families at their homes to review and obtain signatures on paperwork, including but not limited to releases of information, client consents, and treatment plans
  • Works with families in their homes to reinforce skills being taught by therapist in client sessions for families who are unable to attend school based counseling sessions
  • Key duties include coordinating services, providing psychosocial support, and maintaining detailed case records; the School Based Targeted Case Manager will refer clients to appropriate community resources and coordinate with counselors to ensure effective service delivery; this role requires strong problem-solving, communication, and cultural sensitivity skills.
  • Provides support for improved community service and social support referrals & linkages
  • Manages the cycle of transitional care, including appropriate follow-up from inpatient to other settings
  • Provides support in navigating complex health care and social services systems
  • Delivers advocacy for individuals and families through linkages with appropriate level of care after the assessment of needed services
  • If the recipient is eligible, working to secure Medicaid benefits with the goal of subsequent Health Home enrollment
  • Focus on HEDIS, population health standards Social Determinants Measurement to Improve Quality of Care, and Care Transitions.
  • Performs monitoring and follow-up services which may include contact with the service recipient, the recipient's family and/or collaterals, service providers, and/or other entities or individuals.
  • Manages coordination amongst the TCM/Clinic and Care Coordination teams
  • Assessment and reassessment - securing of information through collateral sources of the nature and degree of the consumer's functional behavioral health impairment, eligibility for services, identification of barriers to care and gaps in service, assessment of service needs (including vocational, medical, social, psychosocial, educational, financial and other services), description of strengths, informal support system identification and environmental factors relative to consumer's care.
  • Completes all trainings required by the agency.
  • Performs all other duties as assigned.
QUALIFICATIONS
  • Bachelor's degree in one of the following listed fields: social work, psychology, nursing, rehabilitation, education, occupational therapy, counseling, or community mental health; plus one year experience in providing direct services to mental health and/or substance use disorder consumers -OR- Master's degree in one of the above listed fields may be substituted for one year of experience
  • Bilingual strongly preferred.
  • Requires understanding of mental health and substance use disorder diagnosis and symptomatology
  • Working familiarity with community resources, including entitlement programs, medical, financial and legal services, housing and emergency food programs, and the range of mental health treatment and rehabilitation services available.
  • Must be able to effectively broker services and develop resources.
  • Must be able to perform duties in an independent and efficient manner.
  • Must possess knowledge of and ability to provide crisis intervention strategies effectively.
  • Valid NYS Driver's License.
  • Must have accredited NYS Defensive Driving course; or obtain within 15 days of hire and send certification of completion to Human Resources.
  • Must maintain active NYS Defensive Driving course every 3 years and send a copy of the new certification of completion upon expiration.
  • Must maintain valid vehicle inspection, registration, and insurance cards and send updated copies to Human Resources upon expiration.
