Job description

HOM has a Targeted Case Manager position available in Portland, Augusta, and Lewiston for 28-34 hours/week. $33.00 hourly, plus paid training, meetings, supervision, travel time, and documentation. Employee will receive an additional $2 increase within 2 years.

Requirements:

  • 18 years or older with a bachelor’s degree
  • Reliable transportation with insurance coverage
  • Ability to do a minimum of 28 billable hours weekly
  • Eligibility to work in the USA

Bachelor’s Degree from an accredited four (4) year institution of higher learning with a specialization in psychology, mental health and human services, behavioral health, behavioral sciences, social work, human development, special education, counseling, rehabilitation, sociology, nursing or closely related field,

OR

Bachelor’s Degree from an accredited four (4) year institution of higher learning in an unrelated field and at least one (1) year of full-time equivalent relevant human services experience.

Job Description:

The TCM will assist and support children clients who are diagnosed with a mental health diagnosis. The TCM will identify, support, and strengthen the family’s internal and external resources and help organize them to meet the needs of the child within the family, ensuring follow-through with necessary mental health and medical appointments. The TCM monitors progress and provides accurate and efficient documentation to ensure the children’s needs are reflected.

Additional Tasks:

  • Assess, plan, implement, monitor, and evaluate client’s health and needs
  • Handle case assignments of up to 20 clients
  • Create and implement treatment plans
  • Help clients make informed decisions and advocate for clients
  • Provide education and support to clients and clients’ families
  • Gather and make appropriate referrals for clients
  • Prepare and maintain client records and notes in data EHR
  • Attend necessary trainings and certification courses
  • Ability to travel locally
  • Other duties as assigned
