JOB SUMMARY

This role has primary ownership and oversight of a specific panel of members with varying health statuses and clinical needs. The incumbent assesses health management needs using data, analytics, and clinical judgment to identify appropriate interventions. Supported by a multidisciplinary team, they refer members to relevant resources, manage an active caseload, conduct outreach, develop care plans, and monitor quality outcomes.

ESSENTIAL RESPONSIBILITIES

1. Maintain oversight of members by assessing needs and referring to appropriate resources.
2. Create and manage care plans with goals and regular re-assessment contacts.
3. Achieve targeted patient outcomes and document all activities in compliance with standards.
4. Stay current with applicable regulations and standards for case management.
5. Perform other duties as assigned.

EDUCATION

• Required: High School Diploma/GED
• Preferred: Bachelor's Degree in Nursing

EXPERIENCE

• Required: 7 years of clinical, case management, or related experience
• Preferred: Maternity, NICU, diverse populations experience, cultural competency

LICENSES AND CERTIFICATIONS

• Required: RN license in PA, WV, DE, or NY; others within 6 months
• Preferred: Certification in Case Management

SKILLS

• Strong communication, negotiation, analytical, and organizational skills
• Knowledge of disease processes and healthcare systems
• Proficiency in MS Excel and data interpretation
• Ability to handle multiple priorities and work independently

OTHER INFORMATION

• Travel: 0-25%
• Position: Office-based, teaching/training, occasional travel
• Physical demands include lifting up to 10 pounds rarely; no physical work site required.
