Job Description:

Summary: This role involves managing a panel of members with varying health statuses. The incumbent assesses health management needs, uses data and clinical judgment to identify interventions, and collaborates with a multidisciplinary team to coordinate care and improve health outcomes.

Responsibilities:

• Oversee a panel of members, assessing needs and referring to appropriate resources.
• Create and implement care plans with goals and follow-up schedules.
• Achieve targeted patient outcomes and document activities per standards.
• Maintain knowledge of regulatory and practice standards.
• Perform other duties as assigned.

Education:

• Required: High School Diploma/GED

Preferred: Bachelor's Degree in Nursing

Experience:

• Required: 7 years in clinical, case management, or related fields.

Preferred: Experience with cognitive behavioral therapy, diverse populations, and cultural competency.

Licenses and Certifications:

• Required: RN license in PA, WV, DE, or NY (others within 6 months)

Preferred: Oncology experience and case management certification.

Skills:
