Complex Case Manager RN - Oncology (Remote)

Job Summary

This role involves managing a panel of members with varying health statuses and needs. The incumbent assesses health management needs, utilizes data and clinical judgment to identify appropriate interventions, and refers members to multidisciplinary resources. Responsibilities include developing care plans, encouraging behavioral changes, addressing barriers, coordinating care, and monitoring quality outcomes.

Essential Responsibilities

• Oversee a panel of members, assess needs, and triage to resources.
• Create and implement care plans with goals and regular re-assessment contacts.
• Achieve targeted patient outcome goals.
• Document activities in compliance with standards and regulations.
• Maintain knowledge of applicable regulations and standards.
• Perform other duties as assigned.

Minimum Requirements

Education: High School Diploma/GED. Bachelor's in Nursing preferred.

Experience: 7 years in clinical, case management, or related fields. Preferred experience in cognitive behavioral therapy, motivational interviewing, working with diverse populations, and understanding of cultural competency.

Licenses & Certifications: RN license in PA, WV, DE, or NY required; others within 6 months. Oncology experience and case management certification preferred.

Additional Information

Travel up to 25%. Office-based position. Position involves occasional training, site visits, and lifting up to 10 pounds.

Salary range: $57,700 - $107,800. Compliance with all applicable laws, regulations, and company policies is required.
