Job summary

Mountain BOCES is seeking a SWAP Specialist to provide case management and direct transition services for youth with disabilities in Lake County and Leadville. You will coordinate pre-employment transition services, IEP/ICAP collaboration, and direct service delivery across school and community settings.

The role covers on-site work at Lake County High School and Cloud City High School, with flexible hours including evenings and summers to support direct services and internship opportunities.

Qualifications

• Education: Associate's Degree in related field or equivalent in years of experience in related field

Responsibilities

• Referral and Intake duties: schedule and complete intake, gather DVR paperwork, act as liaison to explain DVR processes and intake requirements.
• Eligibility, Comprehensive Assessment and Plan Development duties: develop Integrated Plan for Employment with services and training, update annually, coordinate linkages to service providers.
• Attend IEP/ICAP meetings to support transition planning and bridge to DVR services.
