Job summary

Harbor House in Central Florida is seeking an Outreach Case Manager to provide direct assistance and case management to survivors of domestic violence through our Outreach Program. You will assess needs, develop service plans, conduct lethality assessments, and connect clients with legal, financial, medical, housing, and educational services.

The role requires strong communication, advocacy, and the ability to work with diverse partners while maintaining confidential records and meeting funding

Qualifications

• Undergraduate degree in Social Work, Psychology, Counseling, or related field preferred.
• Minimum of 1 year of case management experience, preferably in a domestic violence program.
• Florida Driver’s License with a clean record for the past 5 years.
• Strong writing and communication skills; ability to manage multiple priorities.
• CORE Competency training within 90 days of hire; annual mandatory training.

Responsibilities

• Work with survivors to determine needs, set goals, and develop service plans.
• Provide lethality assessments and crisis counseling.
• Advocate and coordinate with legal, financial, medical, housing, and other services.
• Link and refer to community providers; conduct follow-up meetings.
• Document data and maintain client confidentiality; ensure audit readiness.
• Coordinate with Harbor House advocates and shelter staff.
• Facilitate support and life skills groups.
