Job summary

Harbor House of Central Florida is seeking an Outreach Case Manager to provide direct assistance and comprehensive case management to survivors of domestic violence through the Outreach Program. You will assess needs, develop service plans, and maintain confidential records while coordinating with advocates and community partners.

Qualifications

• Bachelor's degree in Social Work, Psychology, Counseling, or related field.
• Minimum of one year of case management experience, preferably in domestic violence.
• Valid Florida Driver’s License with clean record for past 5 years; 21+ per agency requirements.
• Strong written and verbal communication; ability to manage multiple priorities.

Responsibilities

• Provide direct assistance and case management to survivors of domestic violence.
• Conduct lethality assessments and crisis counseling.
• Develop service plans and maintain accurate client/service records for audits.
• Link survivors to community services; perform referrals and follow-ups.
• Coordinate with Harbor House and community agencies to ensure coordinated services.
• Maintain confidentiality and document outcomes for program reporting.
