Qualifications

• Must hold an RN license (IDFPR) or be eligible for licensure in Illinois.
• BLS certification within 30 days of hire (AHA).
• Bachelor's degree in Nursing is required.

Responsibilities

• Promote clinical excellence through nursing process, protocols, and procedures.
• Assess patient status via interview, observation, and physical exams.
• Develop individualized care plans with patient/family and health care team.
• Implement interventions to achieve expected patient outcomes.
• Document patient progress and modify plans as condition changes.
• Ensure safe patient handling and use of lift equipment where applicable.
• Provide patient and family education tailored to age and readiness.
• Collaborate with interdisciplinary team for care coordination from admission to discharge.
• Participate in performance improvement projects and data collection.
