Responsibilities

• Assess patient status through interview, observation, and physical assessment.
• Develop an individualized plan of care with the patient/family and health care team.
• Implement and evaluate care to achieve expected patient outcomes.
• Document patient progress and maintain patient charts.
• Promote patient and staff safety and participate in quality improvement.
