• Develops an individualized plan of care using a collaborative approach with the patient/family and health care team.
• Uses appropriate interventions identified in the plan of care to achieve expected patient outcomes.
• Systematically evaluates and documents patient progress toward outcomes during shifts.
• Modifies plan of care based on evaluation and patient condition.
• Demonstrates equipment knowledge and troubleshoots problems.
• Promotes patient and staff safety through lift equipment.
• Obtains lab specimens and performs bedside lab testing in compliance with orders.
• Coordinates interdisciplinary care and provides goal-focused shift reports.
• Advocates for patients and participates in performance improvement initiatives.
