Job summary

Methodist Le Bonheur Healthcare seeks a Registered Nurse to lead care coordination across the patient journey within our health system. You will assess physical, psychological, and social needs and partner with providers to ensure seamless treatment and follow-up.

You will educate patients and families, document care data, and perform RN clinical procedures. This role requires strong communication, compassion, and teamwork in a busy clinical setting.

Qualifications

• Associates Degree in Nursing is required.
• Bachelor's Degree in Nursing is preferred.
• RN license in Tennessee required.
• BLS certification required.
• Certification in clinical specialty is preferred.

Responsibilities

• Assess the physical, psychological, and social needs of the patient and improve care for the patient and family.
• Coordinate communication with providers on behalf of the patient.
• Develop and coordinate education, resources, and activities for patients and families.
• Document, track and monitor patient data.
• Perform clinical procedures within RN scope and training.
• May perform phone duties within RN role.
• Perform other duties as assigned.
