Job summary

Holy Cross Medical Center is seeking a Patient Navigator (RN or LPN) to support patients and families through the care continuum, coordinating services and addressing barriers to care. The role emphasizes transitions in care, patient advocacy, and education to ensure a seamless experience for patients undergoing procedures or surgeries.

The PN collaborates with physicians, nurses, coordinators, and care teams to improve outcomes and patient satisfaction, leveraging EHRs and communication tools

Qualifications

• Current and valid nursing license in NM or compact state, maintained during employment.
• Minimum 3 years of clinical nursing experience.
• BLS certification required.
• Strong communication and interpersonal skills.
• Knowledge of ambulatory care trends and practices.

Responsibilities

• Coordinate patient care across the continuum through interdisciplinary collaboration.
• Expedite and coordinate care to minimize barriers to services.
• Focus on improving transitions in care and adherence to recommended plans.
• Advocate for patients and educate families and caregivers.
• Build relationships with patients, families, and the care team to deliver seamless care.
