Nurse Navigator (Cardiovascular Surgery)

Coordinate cardiovascular surgical patients across the full care continuum as a central point of contact. Provide comprehensive pre- and post-operative education, conduct patient assessments, and ensure appropriate scheduling of tests, procedures, and follow-up care.

Partner with surgeons, cardiologists, and interdisciplinary teams to streamline transitions of care, improve care coordination, support patient understanding and adherence, and help reduce readmissions while promoting quality clinical outcomes.
