Job summary

Holy Cross Medical Center is seeking an experienced Registered Nurse Navigator for the Surgery service. You will coordinate patient care through multidisciplinary teams, with a focus on seamless transitions and reducing barriers to surgery.

The RN Navigator will educate patients and families, advocate for optimal outcomes, and collaborate with providers to address system barriers. Strong ambulatory care knowledge and EHR proficiency are essential.

Qualifications

• Current and valid nursing license in the State of New Mexico, or compact state; maintained current throughout employment.
• Strong professional, personable and motivational personality that builds strong relationships with patients.
• Strong knowledge and interest in ambulatory care and current techniques.
• Knowledge of Microsoft Office and familiarity with electronic health record systems.
• Ability to establish and maintain effective working relationships with leadership, physicians, and care teams.
• BLS certification is required and maintained throughout employment.

Responsibilities

• Coordinate patient care through interdisciplinary collaboration to achieve optimal outcomes.
• Focus on improving transitions in care and adherence to care plans.
• Act as a clinical advocate and educator for patients and families.
