Job summary

Familylinks, Inc. seeks a Supports Coordinator to locate, coordinate, and monitor services for individuals with intellectual disabilities or autism in Pennsylvania.

You will work with families to ensure access to needed resources and may advance to SC1/SC2/Lead SC, with roles based on education and experience. Starting pay is $20/hour with a monthly bonus potential up to $1,000, and opportunities for career advancement across Carbon and Pike counties.

Qualifications

• Bachelor's degree with 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or related social science.
• 2+ years' experience as a County Social Service Aide 3 and two years of college coursework with 12 credits in related social science.
• Equivalent combination of 12 college credits and 1+ year paraprofessional case management experience.
• 12 college credits and two years of professional experience in developmental disabilities, mental health, or related fields.

Responsibilities

• Complete all duties within regulatory timeframes.
• Document thoroughly after each contact/service.
• Meet goals and report per policy.
• Assist in emergency situations as requested.
• Attend supervision/meetings/training as required.
• Record time and attendance in HRIS/timekeeping systems.
• Report monthly expenses per policy.
• Model the organization's mission and values.
• Other duties as assigned.
