Job summary

Familylinks is seeking four Supports Coordinators to cover Clairton, North Hills, South Hills, and Washington County. The role focuses on locating, coordinating, and monitoring services for individuals with intellectual disabilities and/or Autism, with opportunities to advance to SC1, SC2, or Lead SC.

A monthly bonus up to $1,000 is available and starting pay is $20/hour. Flexibility is required with remote/office/community work; 60% field work and 40% office-based tasks.

Qualifications

• Bachelor's degree with at least 12 credits in sociology, social welfare, psychology, gerontology, criminal justice, or related social science.
• 2+ years' experience as a County Social Service Aide 3 and two years of college coursework including at least 12 credits in related social services.
• Any combination including 12 credits in social sciences and 1+ years of paraprofessional case management experience.
• Combination of 12 credits in related social science and two years' professional experience in developmental disabilities, special education, mental health, counseling psychology, or health and rehabilitative services.
• A valid driver's license and access to a vehicle.
• Proficiency with MS Office, computer/laptop, tablet and cell phone.

Responsibilities

• All essential duties and responsibilities are completed within regulatory timeframes.
• Complete timely and thorough documentation after each contact/service provided.
• Meet productively goals and report accurately per departmental policy.
• As requested, assist in emergency situations.
• Attend supervision/meetings/training as requested/required.
• Accurately record time and attendance via HRIS and Timekeeping system.
• Document and report monthly expenses accurately and within agency policy expectations.
• Model the organization's mission and values.
• Other duties as assigned.
