Job summary

Familylinks seeks a Supports Coordinator to locate, coordinate, and monitor services for individuals with intellectual disabilities and/or Autism. You will collaborate with families and caregivers to ensure access to necessary resources and supports to thrive. Opportunities exist to advance to SC1/SC2/Lead SC across Carbon and Pike Counties.

Starting pay is $20/hour with a potential monthly bonus up to $1,000, and Familylinks emphasizes supervision and ongoing training for certification paths.

Qualifications

• Bachelor's degree or related social science with 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or related field.
• 2+ years' experience as County Social Service Aide 3 and 2 years of college coursework in related fields.
• Equivalent combinations including 12 college credits and paraprofessional case management experience.

Responsibilities

• Complete duties within regulatory timeframes.
• Document after each contact/service.
• Meet productivity goals and report per policy.
• Attend supervision/training as requested.
• Record time and attendance in HRIS/Timekeeping system.
• Report monthly expenses within policy.
• Model the organization's mission and values.
• Other duties as assigned.
