Qualifications

• Bachelor’s degree required; Human Services-related field preferred.
• Strong communication and relationship building skills.
• Experience with case management, social services, or healthcare preferred.
• Excellent organizational and documentation abilities.
• Valid driver’s license, reliable transportation, and auto insurance required.

Responsibilities

• Coordinate services and manage a caseload of individuals with ID/ASD.
• Conduct regular visits to assess health, safety, and service needs.
• Collaborate with families, providers, and community resources.
• Monitor service delivery and address concerns or gaps in care.
• Maintain accurate documentation, reports, and case notes.
