Zachary Piper Solutions is seeking a Supports Coordinator to join a healthcare and social assistance provider in the Chester area of PA. The role manages a caseload of 40-50 individuals, conducts regular monitoring visits, and coordinates transportation, employment support, medical appointments, and community resources.

The position requires strong organization, the ability to document thoroughly, and travel to local sites.

Qualificações

• Experience in human services, case management, or related fields preferred but not required.
• Proficiency with Microsoft Office, including Excel (preferred).
• Strong organization and ability to manage a high-volume caseload.
• Ability to build professional relationships with individuals, families, and providers.

Responsabilidades

• Manage an assigned caseload of approximately 40-50 individuals with intellectual and developmental disabilities.
• Conduct regular in-person monitoring visits with individuals, families, caregivers, and service providers.
• Coordinate and facilitate support services including transportation, employment assistance, job coaching, medical appointments, and community resources.
• Gather, document, and maintain detailed case information with monitoring tools and written reports.
• Perform ongoing follow-up with individuals, families, providers, and community resources to ensure services remain in place.
